Help for sending email 
 
I apologize but I do not know how to define the name of this topic, and I hope to explain myself to the fullest. 
 
So I have 2 files, 1 file is in html and php in 1 file 
 
this is the html file that I enter in a cms called proposed acquisition, which will be the home page

Both files should I put them in the root of ps, so when a customer clicks above must enter their credentials, and send me an email. 
 
I would like to know which command should I enter in the php file or html that once the customer presses send me an e-mail arrives? 
 
I hope I explained myself better
 
Here you can find a demo, but does not send e-mail
