I upgrade from 1.0.0.8 to 1.2.2.0 and I have SQL error

(1062) Duplicate entry ‘postUpdateOrderStatus’ for key 2

And when I want to see pdf for order a see error Hack attempt

According to sticky, there is something wrong with order status.
If you had modified order status, delete all of modification or revert to its original value.

I never touch the order status function so I didn't encounter this kind of problem.
