i am newbie to prestashop 1.5.4 and i want to add custom fields on contact form.tpl

i want to add some fields in prestashop contact-form.tpl 1.5.4 as well as i have tried to take all the fields from address page but nothing going to my database table and there is no required error status showing help me i m in big problem.
