Duplicate Conact Form

I managed with ease to duplicate the contact forms in version 1.4.2.5, however having big probles in 1.4.6.2. I have followed PrestaShop Fanatic's tutorial in the following forum post: http://www.prestashop.com/forums/topic/121132-how-to-create-a-copy-of-contact-form-solved/

 

But all I keep getting is redirected to the standard contact form. Even if I turn off URL re-write and just type in the the new php page (contact-form-parts.php) it still redirects.

 

Any help would be great.

PS I have attached my files. One is the TPL file I had to rename because it did not let me attach.
