Registration required for certain products.

Hi

 

I run a pet supplies website. I was wondering if there is a module to register certain properties for certain products.

 

example.

 

for product A

 

Pet registration is required to purchase the item

 

Require Pet Registration

 

i.e name, weight, sex. something like the image below.

 

Anyone have any ideas if there is such a module available.
