Looking for delivery module with charges for delivery into different floors of the building.

We are doing furniture business. 

So we charge delivery fee (eg. $20) for ground floor and $20  extras for the use of an exterior lift.

user must enter the floor he want to deliver. based on that input fee should be calculated.

is there any module available to calculate based on the user input. ?
