Lastname and Firstname USA address

Hello,

For a few days without knowing why, it is no longer possible to order on my site https://kegs-shop.com with a delivery address in the USA. Indeed, as soon as the customer selects USA as the country of delivery, his name and surname disappear and a "please choose" box appears !! This does not encounter this problem with other countries (nor Canada which also contains states).

If the customer manages to add his address (with the state and USA), the next page will be a 500 error page.
