Customer addresses empty or missing

I noticed that there are a lot of empty addresses in the ps_address table and not all the id_address values are consecutives.

I am using the one page Supercheckout module and the addresses should be recorded when the user is pressing the "Finalize order" button, so I suspect something is happening during the checkout process that is preventing the addresses to be recorded in the database. Thus, the orders might not finalize correctly.

Why are the id_address values not consecutives and how can I fix this?

My version is 1.6.1.19.
