Order Confirmation auto-email not being populated?

A customer has just sent me this copy of the confirmation email he received, it's goosed (personal details changed to protect the innocent)...

Has Mail.php been overwritten ?  No

Error log ? Can't see any obvious errors (BO - 'Configure' - 'Advanced Parameters' - 'Logs'
