IMEI, serial number, problems, accessories) and form send to me a "repair" code and send to client instructions to send the equipment for repair... I dont know the correct name for this service

Im wondering if existe some module or way, for I put online an platform for my clients fill with some characteristics and send the equipment to me for repair after I tell them OK to send... Open a RMA on prestashop I just can work with the client if we buys an item... Witch is not the case...
